WHO IS ELIGIBLE TO BE AN ELDERCARE PATIENT?
Any Medicare-eligible patient living in an assisted living facility or long-term care facility who prioritizes their health and well-being and is willing to pay a monthly membership fee for additional health care services, not typically available in office-based primary care facilities. Patient volume is limited, given the quality of care provided to members.
